Patient Registration


 

To provide you with the best possible care, we need the most up-to-date information about you.  If you are a new patient or an established patient who has not visited our office in three years or more, we will need an updated registration form at your next appointment.

To expedite your visit, please complete our Permission To Treat, Refraction & Contact Lens Evaluation Fee and secure Patient Registration Form before your next appointment.  You can register online in just a few easy steps. If possible, print these three forms and bring them to your appointment.
 
Scheduling Appointments
 
Use our fast and easy Appointment Request Form to request the appointment time, location, and doctor that you prefer.  Once you have submitted your request online, we will contact you with the appointment options that best match your preferences.
 
Your First Visit
 
Your first appointment will take approximately two hours.  Both eyes will be dilated for complete exams, retinal evaluations, and other procedures.  Dilation causes sensitivity to light and an inability to see well at close range.  You should have someone accompany you to drive you home, or you should wait until your eyes return to normal so that you can drive safely.
 
What To Bring To Your Appointment
 
Please plan to arrive early and bring the following items to your next appointment:

             2. Referral Forms (if required)

             3. Permission to Treat 

             4. Refraction & Contact Lens Evaluation Fee

             5. Patient Registration Form (if required)

             6. Any Required Co-payment

After-Hours Emergencies
 
One of our physicians is on call every day.  Please call us immediately at
404-256-1507 if you have an ocular emergency or call 911.  After normal business hours, our answering service will page the physician who will return your call.
